What you need to know
- Not a Driver License: An ID card is for identification only and does not allow you to drive.
- No Age Restrictions: If you are under 16, a parent or guardian must sign for you.
What you need to do
Fill out online application & make appointment
- REQUIRED: Online application
- SAVE TIME: Make appointment
Gather your documents - must bring to appointment
- Proof of identity
- Proof of social security number
- Two (2) documents showing proof of Utah address
- Additional documents for name change (if applicable)
- Get details about acceptable documents
At your appointment
- Have your confirmation number ready to check-in
- Have photo taken
- Present documents
- Pay non-refundable fee of $23.00
- If you are under 16, have parent or guardian sign for you
- A temporary identification card with a photo will be issued on the same day.
- The completed identification card with an expiration date of five (5) years will be mailed to you in approximately 6-8 weeks.
