Required Documentation


To ensure your application process is as smooth as possible, remember to bring all required documents. Faxed or photocopied documents will NOT be accepted for any document. All documents will be imaged and stored in a secure database and returned to the applicant. In some cases, document approval may not occur on the same day and may require an additional visit.

Suppose the name from your driver license or identity card is different than the name on your identity document (e.g., birth certificate or passport.) In that case, it must be accompanied by legal documentation authorizing the name change. These include a certified state marriage certificate, divorce decree, adoption, citizenship, guardianship, or court documents indicating the name change.

A certified translation of the document must accompany non-English documents. The translated document must be translated by one (1) of the division’s approved translators, and the original and the translation are considered one (1) document for identification purposes.
