Replace Adult Limited-Term Identification Card (ID)

1 – WHAT YOU NEED TO KNOW

Lost or Stolen

You may want to report your ID card lost or stolen.  If you have a physical copy of the police report indicating that your ID is being used fraudulently, bring the report with you to the office when you come in to re-apply.

New Address

Utah law requires you to update your address with the Driver License Division within ten (10) days of moving.  The easiest and fastest way to change your address on your driving record is online.  However, if you would like your new address printed on your ID card, you will need to schedule an appointment to come in for a replacement.

Name Change

To change your name on your ID card, legal documentation authorizing the name change is required.  These include a certified state marriage certificate, divorce decree, updated passport, amended birth certificate, adoption paperwork, citizenship/naturalization certificate, or court documents indicating the name change sealed by the court.

2 – WHAT IS THE PROCESS

  • Fill out an online application and schedule an appointment to visit a driver license office.
  • When you arrive for your scheduled appointment, have your confirmation number ready to check in, and then you will take a photo.
  • After your photo, you will have a seat; when your number is called, you will present your documents to the examiner, and pay the non-refundable fee of $23.00.

3 – WHAT YOU NEED TO BRING TO YOUR APPOINTMENT

  • Bring your current Utah limited-term ID card or something with your name on it.
  • Bring your current lawful presence identity document.
    • A S.A.V.E. verification will be run to verify your document status (this process may take up to 14 days.)  To understand S.A.V.E verifications, click here.
  • Bring two (2) documents providing proof of your Utah address if you have moved from the last known address on your ID card. You can avoid providing proof if you submit your address change online prior to your scheduled appointment.
  • For a name change, bring the authorized documents listed above.
  • If homeless, click here.

4 – COMPLETE THE APPLICATION AND SCHEDULE AN APPOINTMENT

5 – WHEN DO I GET MY ID CARD

  • A temporary limited-term identification card with a photo will be issued on the same day.
  • The completed identification card will have an expiration date of your legal presence document or five (5) years, whichever is sooner; or 1-year from the date of issuance if there is no expiration date, and will then be mailed to you in approximately 4-6 weeks.

Note: By law, you may not possess more than one REAL ID compliant certificate at a time (e.g., a Utah driver license and a Colorado ID.)
