Ignition Interlock Device Provider


  • Ignition Interlock Manufacturer  – business entity that produces ignition interlock devices. 
  • Ignition Interlock Service Center – a physical structure where interlocks are installed, serviced, and removed. 
  • Installer – individual who performs the installation, servicing, and removal of the ignition interlock. Click here for a list of installers.
BECOMING A MANUFACTURER PROVIDER

Step 1: Read Utah Administrative Rule R708-31 and R708-48.
Step 2: Submit a provider application and fee.
Step 3: Submit an application for the provider representative, service centers, installers, and fees (the provider is responsible for all costs associated with the check.)
Step 4: Submit a fingerprint card for the provider representative and installers for a criminal background check (the provider is responsible for all costs associated with the check.)
Step 5: Submit service center business license.
Step 6: Submit any forms, receipts, schedule of fees and contracts that will be used.
Step 7: Submit an installer training plan.
Step 8: Submit a client training plan on how to use and maintain the system.
Step 9: Submit evidence of a $50,000 surety bond.

Once the provider has submitted this information, the Driver License Division will review all the information for accuracy and provide any feedback for changes if necessary.  Once all background checks have been completed, all installers must pass a training quiz regarding the rules and regulations of the ignition interlock program.  The Driver License Division will then conduct a site inspection of the service center to ensure that the location meets the administrative rule requirements.

Fees made payable to the Department of Public Safety:

  • Manufacturer: $100
  • Representative: $30
  • Installer: $30
  • Service Center: $30
  • Fingerprints: $15 (separate fee made payable to BCI)
