Invisible Condition


This voluntary program allows a symbol “sticker” to be placed on your driver license or identification card upon request. The invisible condition is used to notify a law enforcement officer or other first responders of a physical or mental health condition that may interfere with the ability to communicate with others. For example:

  • Communication impediment
  • Hearing loss
  • Autism spectrum disorder
  • Drug allergy
  • Alzheimer’s disease or dementia
  • Post-traumatic brain injury
  • Schizophrenia
  • Epilepsy
  • Developmental disability
  • Down syndrome
  • Diabetes
  • Heart condition
  • Other conditions identified by a healthcare professional

To qualify, you must submit the completed Invisible Condition Request form, signed by your health care professional to the Driver License Division by mail, fax, email, or office location. The division will add the information to your record and provide you with a symbol.

The symbol may be removed from your record by completing the Invisible Condition Removal form.

FAQs

Who can see the symbol?
The symbol is a sticker that is attached to your identification card or driver license. If you display your license or ID card others will see the symbol.

Where do I place the sticker on my license or ID card?
It is recommended to place the sticker after the address line on the front of the card.

How do I get a new sticker if I lose mine?
You can get a duplicate sticker by contacting the medical section by phone at 801.963.7325, email at dlmedical@utah.gov, or mail to PO BOX 144501, SLC, UT 84114.

How do I get a new sticker when I renew?
Once you renew your license or identification card, a letter is sent with a new sticker to be placed on the card.
