Replace Adult Limited-Term Commercial Driver License (CDL)


1 – WHAT YOU NEED TO KNOW

When applying for a replacement CDL, the expiration date of the new license will be the same as your current license.  However, if your license expires in the next six (6) months, you are eligible to renew your commercial driver license.

Lost or Stolen

You may want to report your commercial driver license lost or stolen.  If you have a physical copy of the police report indicating that your license is being used fraudulently, bring the report with you to the office when you come in to re-apply.

New Address

You are required to change your address on your license no later than 30 days after moving.  You will not be able to change the address online as Federal regulations require that your current address be printed on your CDL.  Additionally, you will be required to provide proof of your Utah address.

Name Change

To change your name on your driver license, legal documentation authorizing the name change is required.  These include a certified state marriage certificate, divorce decree, updated passport, amended birth certificate, adoption paperwork, citizenship/naturalization certificate, or court documents indicating the name change sealed by the court.



2 – WHAT IS THE PROCESS

  • Fill out an online application and schedule an appointment to visit a CDL driver license office.
  • When you arrive for your scheduled appointment, have your confirmation number ready to check in, and then you will take a photo.
  • After your photo, you will have a seat; when your number is called, you will present your documents to the examiner, take an eye test, and pay the non-refundable fee of $23.00.

3 – WHAT YOU NEED TO BRING TO YOUR APPOINTMENT

  • Bring your current Utah limited-term commercial driver license if you have it or something with your name on it.
  • Bring your current lawful presence identity document.

  • Bring two (2) documents providing proof of your Utah address if you have moved from the last known address on your Utah commercial driver license.  Federal regulations require that your current address be printed on your limited-term commercial driver license.
  • For a name change, bring the authorized documents listed above.

4 – COMPLETE THE APPLICATION AND SCHEDULE AN APPOINTMENT

Currently, our Farmington, Logan, and Tooele offices do not process CDL’s.


5 – WHEN DO I GET MY LICENSE

  • A temporary license with a photo will be issued on the same day.  
  • The completed limited-term CDL will have an expiration date of your legal presence document or five (5) years, whichever is sooner; or 1-year from the date of issuance if there is no expiration date, and will be mailed to you in approximately 4-6 weeks.

Note: By law, you may not possess more than one REAL ID compliant certificate at a time (e.g., a Utah driver license and a Colorado ID.)
