Replace Minor Driver License


1 – WHAT YOU NEED TO KNOW

When applying for a replacement license, the expiration date of the new license will be the same as your current license.  However, if your license expires in the next six (6) months, you are eligible to renew your driver license.

Lost or Stolen

You may want to report your driver license lost or stolen.  If you have a physical copy of the police report indicating that your license is being used fraudulently, bring the report with you to the office when you come in to re-apply.

New Address

Utah law requires you to update your address with the Driver License Division within ten (10) days of moving.  The easiest and fastest way to change your address on your driving record is online.  However, if you would like your new address printed on your license (not required), you will need to schedule an appointment to come in for a replacement.

Name Change

To change your name on your driver license, legal documentation authorizing the name change is required.  These include a certified state marriage certificate, divorce decree, updated passport, amended birth certificate, adoption paperwork, citizenship/naturalization certificate, or court documents indicating the name change sealed by the court.

Attention: The parent or legal guardian who signed for your original driver license does not need to sign again for a replacement driver license unless there was a signature withdrawal from the parent/legal guardian.

At any time, the parent or legal guardian who signed for financial responsibility on the minor’s application can withdraw their signature until the minor is 18 years of age and for any reason.  This action would invalidate the driver license, and the minor could reapply when they reach the age of 18 or if the parent/legal guardian signed a new application.  There would be a licensing fee and a written knowledge test to reapply.


2 – WHAT IS THE PROCESS

  • Fill out an online application and schedule an appointment to visit a driver license office.
  • When you arrive for your scheduled appointment, have your confirmation number ready to check in, and then you will take a photo.
  • After your photo, you will have a seat; when your number is called, you will present your documents to the examiner, take an eye test, and pay the non-refundable fee of $23.00.

3 – WHAT YOU NEED TO BRING TO YOUR APPOINTMENT

  • Bring your current Utah license if you are changing the information on it (i.e., address, name, etc.)
  • If your license has been lost or stolen bring some form of identification/documentation with your name on it.
  • Bring two (2) documents providing proof of your Utah address if you have moved from the last known address on your Utah license.  You can avoid providing proof if you submit your address change online prior to your scheduled appointment.
  • For a name change, bring the authorized documents listed above.

4 – COMPLETE THE APPLICATION AND SCHEDULE AN APPOINTMENT


5 – WHEN DO I GET MY LICENSE

  • A temporary license with a photo will be issued on the same day.  
  • The completed license with the same expiration date as your previous license will be mailed to you in approximately 4-6 weeks.

Note: By law, you may not possess more than one REAL ID compliant certificate at a time (e.g., a Utah driver license and a Colorado ID.)
