- Step 1: Read administrative rule R708-31 and R708-48.
- Step 2: Submit:
- Provider application and fee.
- Application for the provider representative, service centers, installers, and fees (the provider is responsible for all costs associated with the check.)
- Fingerprint card for the provider representative and installers for a criminal background check (the provider is responsible for all costs associated with the check.)
- Service center business license.
- Forms, receipts, schedule of fees, and contracts that will be used.
- Installer training plan.
- Client training plan on how to use and maintain the system.
- Evidence of a $50,000 surety bond.
- Step 3: Complete a training quiz regarding rules and regulations of the IID program.
- Step 4: DLD will conduct a site inspection of the service center to ensure the location meets the administrative rule requirements.
Once the provider has submitted this information, the Driver License Division will review all the information for accuracy and provide any feedback for changes if necessary.
