U.S. Citizen

Category 1: Identity and Date of Birth

One of the following:
  • Valid, unexpired U.S. passport or passport card.
  • A certified copy of a birth certificate filed with the State Office of Vital Statistics or equivalent agency in your state of birth (small laminated birth certificates or hospital certificates are not accepted.)
  • Consular Report of Birth Abroad (CRBA) issued by the U.S. Department of State, form FS-240, DS1350, or FS-545.
  • Valid, unexpired permanent resident alien card, conditional permanent resident card, form I-551.
  • Certificate of naturalization issued by DHS, form N550 or N-570.
  • Certificate of citizenship issued by DHS, form N-560 or N-561.

Category 2: Social Security Number

One of the following:
  • Social security card issued by the U.S. government that has been signed (stub will not be accepted.)
  • If the social security card is not available, you may present one of the following documents that includes your name and full social security number:
    • W-2 form
    • SSA-1099 form
    • Non SSA-1099 form
    • Pay stub
  • Letter from the Social Security Administration indicating ineligibility to obtain a social security number as a result of lawful presence status.

Category 3: Utah Address

Two of the following displaying your name/address and dated within 90 days:
  • Bank statement
  • Court documents
  • Current mortgage or rental contract
  • Major credit card bill
  • Property tax notice
  • School transcript
  • Utility bill
  • Vehicle title
