Documents
You will need to provide 4 documents from the following 3 categories.
Translation
Foreign documents must be translated to English by a certified translator.
Category 1: Identity and Date of Birth
One of the following:
- Valid, unexpired U.S. passport or passport card.
- A certified copy of a birth certificate filed with the State Office of Vital Statistics or equivalent agency in your state of birth (small laminated birth certificates or hospital certificates are not accepted.)
- Consular Report of Birth Abroad (CRBA) issued by the U.S. Department of State, form FS-240, DS1350, or FS-545.
- Valid, unexpired permanent resident alien card, conditional permanent resident card, form I-551.
- Certificate of naturalization issued by DHS, form N550 or N-570.
- Certificate of citizenship issued by DHS, form N-560 or N-561.
Category 2: Social Security Number
One of the following:
- Social security card issued by the U.S. government that has been signed (stub will not be accepted.)
- If the social security card is not available, you may present one of the following documents that includes your name and full social security number:
- W-2 form
- SSA-1099 form
- Non SSA-1099 form
- Pay stub
- Letter from the Social Security Administration indicating ineligibility to obtain a social security number as a result of lawful presence status.
Category 3: Utah Address
Two of the following displaying your name/address and dated within 90 days:
- Bank statement
- Court documents
- Current mortgage or rental contract
- Major credit card bill
- Property tax notice
- School transcript
- Utility bill
- Vehicle title
