U.S. Citizen, U.S. National and Permanent Resident Alien


IDENTITY

Legal/lawful status verification
MUST PROVIDE ONE (1) OF THE FOLLOWING:
  • Valid, unexpired U.S. passport or passport card.
  • Certified copy of a birth certificate filed with the State Office of Vital Statistics or equivalent agency in your state of birth (small laminated birth certificates or hospital certificates are not accepted.)
  • Consular Report of Birth Abroad (CRBA) issued by the U.S. Department of State, form FS-240, DS1350, or FS-545.
  • Valid, unexpired permanent resident alien card, conditional permanent resident card, form I-551.
  • Certificate of Naturalization issued by DHS, form N550 or N-570.
  • Certificate of Citizenship issued by DHS, form N-560 or N-561.

SOCIAL SECURITY CARD

MUST PROVIDE ONE (1) OF THE FOLLOWING:
  • Social security card issued by the U.S. government that has been signed (stub will not be accepted.)
  • If the social security card is not available, you may present one of the following documents that includes your name and full social security number:
    • W-2 form
    • SSA-1099 form
    • Non SSA-1099 form
    • Pay stub
  • Letter from the Social Security Administration indicating ineligibility to obtain a social security number as a result of legal/lawful presence status.

RESIDENCY

MUST PROVIDE TWO (2) OF THE FOLLOWING:
  • Bank statement
  • Court documents
  • Current mortgage or rental contract
  • Major credit card bill
  • Property tax notice
  • School transcript
  • Utility bill
  • Vehicle title

Residency is required for all originals, and if the address is different from what is on the current license record for renewals and replacements.
Documentation must display your name and address, and the verification must be dated within 90 days.
