What is the best way to submit a medical or vision form?
There are three ways you can submit a form. The preferred method is through the Medical Portal. Forms can also be faxed to 801.957.8698, or emailed to [email protected].
Who should I refer my patient to if they have questions about if their form was received?
Driver License customer service at 801.965.4437 or toll-free at 888.353.4224 Monday-Friday, 8:00 am - 5:00 pm (excluding holidays.)
Does my patient need to sign the form?
No, signing the form helps us determine who the form belongs to; it is not required.
Can I use a generic form even though my patient's information is not on it?
Yes, please be sure to include your patient's name and date of birth on it; we will accept it.
My patient says I am supposed to mark a specific category, but he does not know which one.
We recommend completing each category you are following.
Can I recommend a specific review time?
Yes, please indicate the suggested review period in the space provided.
Can I recommend that someone see another health care professional?
Yes.
How long does it take for a medical or vision form to be added to a driving record?
If it is submitted through the Medical Portal, the forms may be added the same day. Email and fax have a turnaround time of 2-3 business days after receipt.
I have a patient who is not safe to drive, so how do I notify the division that they should stop driving?
A HCP may submit a medical form marked level 8, or a vision form marked level 10, or submit a dangerous driver form. The dangerous driver form does not need to be notarized when it is filled out by an HCP.
I have a question or a recommendation for the guidelines, how do I contact the Medical Advisory Board?
The Medical Advisory Board holds public meetings throughout the year.
Can I fill out my own form?
No, a health care professional may not fill out the form for themselves.
