My Record

  • Complete form DLD60m for your regular or commercial driver license (CDL) and submit payment of $8.00.  This form requires your signature to be notarized and dated within 90 days of the date the form is submitted.
  • If you would like a certified driving record, complete forms DLD60m and DLD266M and submit a payment of $10.75.  You must specify the records requested on the form.  A certified record may take two or more weeks to process.

Employer/Insurance Agency/Other

  • Complete form DLD60b if you are an employer, potential employer, insurance agency, or other requester who needs a driver's regular or commercial driver license (CDL) driving record and submit payment of $8.00.
  • The driver's signature must be notarized and dated within 90 days of the date the form is submitted.
  • Any requestor who qualifies under permissible use 1, 4, 6, or 8, please use form DLD60a.
