Complete form DLD60m for your regular or commercial driver license (CDL) and submit payment of $8.00.  This form requires your signature to be notarized and dated within 90 days of the date the form is submitted.  If you would like a certified driving record, complete forms DLD60m and DLD266M and submit a payment of $10.75.  You must specify the records requested on the form.  A certified record may take two or more weeks to process.

EMPLOYER / INSURANCE AGENCY / OTHER

Complete form DLD60b if you are an employer, potential employer, insurance agency, or other requester who needs a driver's regular or commercial driver license (CDL) driving record and submit payment of $8.00.  The driver's signature must be notarized and dated within 90 days of the date the form is submitted.  Any requestor who qualifies under permissible use 1, 4, 6, or 8, please use form DLD60a.
