CDL MVR
CDL records can only be released to you as the driver or the employer.
Appointment Required
To obtain your MVR in an office, you will need to schedule an appointment.
Permissible Uses
The requester must qualify under the permissible uses of the DPPA to obtain an MVR.
My Record
- Schedule an appointment for your visit.
- Bring the completed form DLD60 to your appointment.
- Submit a nonrefundable fee of $8.00.
Record for Someone Else
- Schedule an appointment for your visit.
- Bring the completed form:
- Regular driver: Form DLD60b (driver’s signature must be notarized.)
- CDL driver: Employers or Motor Carriers use form DLD60c.
- Other: Requestors that qualify under permissible use 1, 4, 6, or 8 use form DLD60a.
- Submit a nonrefundable fee of $8.00.
