CDL MVR
CDL records can only be released to you as the driver or the employer.
Appointment Required
To obtain your MVR in office, schedule an appointment first.
Permissible Uses
The requester must qualify under the permissible uses of the DPPA to obtain an MVR.
MY RECORD
- Step 1: Schedule an appointment for your visit.
Step 2: For your regular or CDL driving record, complete form DLD60 and bring it with you to your appointment.
Step 3: Submit a nonrefundable fee of $8.00 at the time of your appointment.
SOMEONE ELSE'S RECORD
- Step 1: Schedule an appointment for your visit.
Step 2: Bring the necessary form:
**Regular driver: Complete form DLD60b (driver’s signature must be notarized.)
**CDL driver: Employers or Motor Carriers request a CDL driving record by completing form DLD60c.
**Other: Requestors that qualify under permissible use 1, 4, 6, or 8 use form DLD60a.
Step 4: Submit a nonrefundable fee of $8.00 at the time of your appointment.
