Unsafe Driver Reporting

A family member, neighbor, or concerned citizen who becomes aware of a physical, mental, or emotional impairment that may present a threat to an individual or others on the highways may submit this Unsafe Driver Reporting form to the Driver License Division.  This form can also be used to report drivers who may be unsafe to drive due to frequent accidents, frequent disregard for traffic laws, or any driving actions observed that may affect driving safety.

The division will evaluate the form to determine if further action should be taken.  Which could include:
  • The driver may be required to complete a driver review, which includes a written knowledge test, a vision exam, and a driving skills test.
  • The driver may also be required to submit a Functional Ability Evaluation Medical Report form or a Visual Examination form completed by their health care professional or vision specialist.

Following the review process, the driver may be granted a restricted driving privilege, or their driving privilege may be denied if they cannot meet minimum testing standards.  If you have more questions, please call our customer service at 801.965.4437 or toll-free at 888.353.4224 Monday-Friday, 8:00 am - 5:00 pm (excluding holidays.)

  • When submitting the form, you must be as detailed as possible.  Include additional pages if necessary.
  • The form must be notarized.  The division is unable to process the review if this is not completed.
  • The division will not disclose the identity of the person notifying the Driver License Division.
  • Under UCA 76-8-504 it is a misdemeanor to knowingly give a false written statement.
