Documents
You will need to provide 6 documents from the following 4 categories.
Translation
Foreign documents must be translated to English by a certified translator.
Category 1: Identity and Date of Birth
One of the following:
- Foreign birth certificate or unexpired foreign passport, including a certified translation if the document is not in English.
- Unexpired Employment Authorization (EAD) issued by DHS.
One of the following:
- Church records
- Court records
- Driver license
- Employee ID
- Insurance ID card
- Matricular consular card (issued in Utah)
- Mexican voter registration card
- School records
- Utah Driving Privilege Card (DPC)
Category 2: Social Security Number or Individual Tax Identification Number (ITIN)
One of the following:
- Social security card issued by the U.S. government that has been signed (stub will not be accepted.)
- If the social security card is not available, you may present one of the following documents that includes your name and full social security number:
- W-2 form
- SSA-1099 form
- Non SSA-1099 form
- Pay stub
- Individual Tax Identification Number (ITIN) issued by the Internal Revenue Service (IRS).
- Document or letter from the IRS verifying the ITIN.
Category 3: Utah Address
Two of the following displaying your name/address and dated within 90 days:
- Bank statement
- Court documents
- Current mortgage or rental contract
- Major credit card bill
- Property tax notice
- School transcript
- Utility bill
- Vehicle title
Category 4: Fingerprint Card and Photo
- You will need to have your fingerprints and photo taken by an Approved Vendor or Law Enforcement Agency.
- Bring the fingerprint card and photo to your appointment.
