MVR in Person


Process for obtaining your MVR in person:

  1. Schedule an MVR appointment for your visit.
  2. Fill out form DLD60 for a regular or CDL driving record and bring it with you to your appointment.
  3. Submit a nonrefundable fee of $8.00 at the time of your appointment.

Certified MVR may only be requested by mail.

To obtain a driving record for someone other than yourself in person, you must qualify under the permissible uses outlined in DPPA. Use form DLD60b for a regular driver (driver’s signature must be notarized.) CDL driving record can only be released to the driver or the employer. Employers or motor carriers can request the CDL driving record by using form DLD60c.  For requestors that qualify under permissible use 1, 4, 6, or 8, please use form DLD60a.
