Have You Recently Changed Your Name?


If you are changing the name from what is on your current Utah driver license or identification card, you must provide legal documentation authorizing the name change. 

If you have had multiple name changes you must have documentation that links all previous names to your current document. Example: Your birth certificate and current marriage certificate should have the same name, if the marriage certificate has a previous married name, you need documentation authorizing that name change.

  • If your current Utah license expires within the next six (6) months, you will renew.
  • If your current Utah license does not expire within the next six (6) months, you will apply for a replacement.

  • If you are applying for your first-time Utah driver license or identification card and your name is different from what is on your identity document (e.g.; birth certificate, passport), you must have legal documentation authorizing the name change.

Acceptable documents include a certified state marriage certificate, divorce decree, updated passport, amended birth certificate, adoption paperwork, citizenship/naturalization certificate, or court documents indicating the name change sealed by the court.
