Name Change


Have you recently changed your name and you would like a new driver license or identification card?

If you are changing the name from what is on your current Utah driver license or identification card, you must provide legal documentation authorizing the name change.  These include a certified state marriage certificate, divorce decree, adoption, citizenship, guardianship, or court documents indicating the name change.

If your current Utah driver license will expire within the next 6 months, you will want to renew your driver license. If your current Utah driver license does not expire within the next 6 months, you will want a replacement driver license. If you need to renew/replace an identification card, click here.

If applying for a Utah original driver license or identification card and your name is different from what is on your identity document (e.g.; birth certificate, passport), you must have legal documentation authorizing the name change. These include a certified state marriage certificate, divorce decree, adoption, citizenship, guardianship, or court documents indicating the name change.
