What you need to know
- Replacement CDL: the expiration date of the new license will be the same as your current license.
- If your CDL expires in the next six (6) months, you are eligible to renew it.

Lost or stolen
- You may want to file a police report if your CDL has been stolen.
- If you have a physical copy of a police report indicating that your license is being used fraudulently, bring the report with you to the office.

Documents for name or address change
- Bring required documents proving name and/or address change, if applicable

- Currently, our Farmington, Logan, and Tooele offices do not process CDLs.
Gather your documents - must bring to appointment
- Bring your current Utah commercial driver license, if it isn't lost or stolen.
- Has your address changed? If yes, bring 2 documents providing proof of your Utah address.
- Federal regulations require that your current address be printed on your commercial driver license.
- Has your name changed? If yes, bring proof of your new name.
At your appointment
- Have your confirmation number ready to check-in
- Have photo taken
- Present documents
- Pass eye test
- Pay non-refundable fee of $23.00

When do I get my CDL?
- A temporary CDL with a photo will be issued on the same day.
- The completed CDL with the same expiration date as your previous CDL will be mailed to you in approximately 6-8 weeks.
