Replace driving privilege card (DPC)
to get new card for name/address change or lost/stolen
What you need to know
- Replacement DPC: the expiration date of the new DPC will be the same as your current DPC.
- If your DPC expires in the next six (6) months, you are eligible to renew it.

Lost or stolen
You may want to file a police report if your license has been stolen.
If you have a police report, bring it with you.
If your DPC was lost, bring something with your name on it to the office.

Documents for name or address change
Bring required documents proving name and/or address change, if applicable

Gather your documents - must bring to appointment
- Bring your current DPC (if it isn't lost or stolen) or something with your name on it
- Has your address changed? If yes, bring two documents showing proof of your new address OR update your address online before your appointment
- Has your name changed? If yes, bring proof of your name change
At your appointment
- Have confirmation # ready to check-in
- Get your picture taken
- Show documents (if address or name has changed)
- Pass eye test
- Pay non-refundable fee of $23

When do I get my license?
- A temporary DPC with a photo will be issued on the same day.
- The completed DPC will have an expiration date of your legal presence document or five (5) years, whichever is sooner; or 1-year from the date of issuance if there is no expiration date, and will be mailed to you in approximately 6-8 weeks.
