DUI Hearing Request

If you have been arrested for driving under the influence (DUI), you have 10 days to request a hearing with the Driver License Division.  Please complete this form to submit a request for a hearing.  If you need to submit additional information with the request, please attach documentation by selecting “Click to Attach File” and uploading the file from your computer.  Once the form has been received, you will be contacted by the Driver License Division to schedule a date and time.

If your hearing request is submitted more than 10 calendar days after the date of arrest, complete this form, attach supporting documentation by selecting “Click to Attach File”, and upload a file from your computer explaining:

1. Why your hearing request is late.
2. Why a late hearing should be granted.
3. Why you believe you will prevail at the hearing.
